Learning Disability and Autism Programme
Are you autistic, have a learning disability or both?
Are you also experiencing a mental health deterioration that may increase the risk of an admission to a mental health hospital?
If you have answered YES, you may be able to get some additional help and support through the Learning Disability and Autism Programme.
Our team’s purpose is about making health and care services better so that more people with a learning disability and/or autism can live in the community, with the right support, and close to home.
As part of our offer, we have the Dynamic Support Register (DSR). This aims to support you or someone you care for to get the right care and treatment, and wherever possible, avoiding the need for a mental health hospital admission.
All individuals on the register will be discussed by a group of professionals from across health and social care who may recommend some additional support for you. This may include:
- A Community Care (Education) and Treatment Review (known as a C(E)TR)
- Referral to the Learning Disability and Autism Keyworker Programme (up to age 25)
- Working with you and those who care for and support you

If you are referring someone, there is an expectation that you provide regular updates of risk of admission so we can ensure we are offering the best possible service.
Would you like to refer yourself or a family member to the DSR? Please complete the online form below.
If you are making a referral please include contact details for your GP, Social Worker or anyone else working with you so we can talk with them about your needs.
We are not a crisis service. We work 9am-5pm, Monday to Friday. We are not always able to respond immediately to queries or referrals.
