What is the Fast Track Tool?
The Fast Track Tool is used to quickly confirm eligibility for NHS Continuing Healthcare funding when someone has a rapidly deteriorating condition that may be entering a terminal phase. It ensures urgent care and support can be arranged without delay.
Who completes the Fast Track Tool?
An appropriate clinician, such as a doctor or nurse, will complete the Fast Track Tool. They will then send it directly to the ICB.
What happens after the Fast Track Tool is submitted?
Once the Hampshire and Isle of Wight Integreted Care Board (HIOWICB) receives the tool, we will arrange a care package to meet your needs as quickly as possible. Our team will also stay in contact with you or your family to ensure the care is suitable.
Will my care be reviewed?
Yes, after few weeks, a review of your care needs will take place. If your condition remains rapidly deteriorating, the review may be delayed, and your care will continue without interruption.
What happens if my condition stabilises?
If your condition stabilises, a Multi-Disciplinary Team (MDT) meeting will be arranged to assess your ongoing eligibility through National Decision Support Tool (DST) which will determin if you still qualify for NHS Continuing Healthcare.
Is NHS Continuing Healthcare free?
Yes, all care provided through the Fast Track Tool and NHS Continuing Healthcare is completely free as part of your NHS treatment.
