The Isle of Wight Health and Care Partnership lead the delivery of care and support services on the Isle of Wight, supporting local people to live healthy and independent lives.
Their upcoming public meeting aims to promote public engagement in the development of strategies and plans that deliver high quality, sustainable and affordable health and care, with a particular focus on the four overarching priority areas contained in the Isle of Wight Health and Care Plan:
- Preventing ill health
- Partnerships
- Productivity
- Pathways (a clinical tool used for assessing, triaging and directing the public to the correct services)
