To let us know that you are interested in working with Hampshire and Isle of Wight ICB please download and review the NHS Standard Contract and provide a breakdown of your rates per service type using the Rate Card Template. You can then contact Hampshire and Isle of Wight ICB by completing the webform below and sending us a completed copy of the Rate Card Template.

Rate Card Template

When completing the Rate Card Template, please follow the steps below:

  • Complete the boxes highlighted in yellow. Please note that Name of Agency, Address, Postcode, Telephone, Email Address, Contact Name at the agency, CQC Registration Link and Localities that you cover only need to be completed on the ‘Hourly Rate Card’ tab as this information will then automatically populate on all the other tabs. Please ensure that the Contact Name, Telephone number and Email Address are for the person who will be receiving the referrals.
  • When completing the ‘localities that you cover’ field, please clearly indicate the areas that you can deliver care, postcodes would be ideal. PLEASE NOTE: that you will only receive referrals in areas that you have specified that you can cover.
  • Please add your rates for each service type that you can provide, by selecting the relevant tab (i.e. ‘Hourly Rate Card’ for domiciliary drop-in, ‘Complex Care – HCA’ for complex care delivered by an HCA etc). Our Commissioning team will then review these rates and compare them to the average rates that we pay for comparable services.
  • If your agency can provide Complex Care, please show the types of complex care need that you can support by selecting ‘Yes’ or ‘No’ for each condition in the boxes shown at the top right-hand corner of the Complex Care tabs.

Once all the required information has been supplied and rates have been approved by AACC Commissioning Team, your agency will become an approved provider and added to our Provider Directory, making you visible to our locality teams and available to receive NHS referrals.
