Welcome to the Population Health Communities of Practice

A vibrant and collaborative space dedicated to fostering dialogue, sharing insights, and promoting collective learning in the dynamic field of population health. The Population Health Communities of Practice spaces have been established in response to recognising the need for a platform where professionals and practitioners can come together to learn as communities of practice, exchanging knowledge and ideas and growing and developing their skills to drive positive change in health outcomes for our populations and communities.

Below are some existing Communities of Practice in place to support you with your work. You are welcome to join any of these. More Hampshire and Isle of Wight Communities of Practice will be coming soon.

Coordinated by Community First, the Hampshire and Isle of Wight Social Prescribing Network was established in 2017. There are three free Webinars and one face to face networking and showcase meeting per year, as well as interim Social Prescribing updates and news. The Network is funded by Hampshire and Isle Wight Integrated Care Board and has c400 members who deliver, commission, or provide services for or support social prescribing projects countywide.
