A collaboration to implement sustainable care for chronic kidney disease, in the areas of Hampshire and Isle of Wight with greatest need.
This collaborative working project between Boehringer Ingelheim Limited (BIL) and NHS Hampshire and Isle of Wight aims to make it easier for everyone to access the treatment that they need and receieve better, more effective care. It particularly aims to support people from communities with a higher risk of chronic kidney disease (CKD) and communities where accessing the right care can be more challenging.
The project focuses on reducing health inequalities by improving early diagnosis and providing proactive care to slow the progression of CKD.
Key Objectives:
- Increase CKD awareness among at-risk populations
- Improve opportunities for early detection and management of CKD
- Provide evidence-based care to all individuals, regardless of demographics
- Enhance the skills and confidence of NHS staff in managing CKD
- Ensure long-term sustainability of CKD services.
- Support the implementation of the recommendations from the Renal Service Transformation Programme
This project began in December 2024 and is anticipated to run for 24 months.
This collaboration between Boehringer Ingelheim Ltd and NHS Hampshire and Isle of Wight to design and deliver a new model of care, will provide innovative approaches to improve healthcare for patients at risk of, or diagnosed with CKD.
