Guidance and resources to connect with your community

Connecting to support in the local community is important to help people stay well and thrive. Here are five steps to help tackle social isolation with links to resources and local support services. 

Spot the signs

Loneliness is a part of life and we're all affected by it. But there are times when people may be more at risk. This can include after a stay in hospital, becoming a new parent or leaving home to go to university. You can recognise the signs and symptoms in yourself and those around you with this advice from NHS Every Mind Matters.

It’s good to talk

Website, text and telephone support lines can offer friendship, conversation and support 24/7. Call the Silver Line from Age UK on 0800 4 70 80 90 or text SHOUT to 85258.

NHS and Care Volunteer Responders can help with check in and companionship calls too. Visit their website or call 0808 196 3646 if you'd like to request support. Healthcare professionals can refer people for additional support such as with the delivery of medication and support with essential shopping.

Social Prescribers

Ask your GP practice about an appointment with a Social Prescriber. They are experts at helping patients connect with support in the community. You can find out more about Social Prescribing on the NHS England website

Financial, cost of loving and other support

There is support available for people struggling with fuel or water poverty, or other cost of living issues. People leaving hospital can be referred to the Home and Well service. Use the links below for cost of living support in your local area.
